Responsibilities

Lead and direct all quality management activities for the organization. Create a robust quality plan annually, lead quality training, collect data on annual quality plan initiatives, analyze quality reports for missed opportunities to develop and implement strategies to mitigate gaps in care by streamlining clinic processes. Regularly presents performance results to staff, leadership and governance. Ensure patients receive the highest quality care, and promote a culture of prevention throughout the organization by promoting quality as a crucial function of all staff members.

  • Promote the mission and philosophy of providing the highest quality healthcare and related services to the medically underserved
  • Ensure compliance, accreditation standards and documentation are met in all relevant areas of responsibility, i.e., HEDIS and UDS
  • Collaborate with organizational stakeholders to develop and implement analytic programs to capture data and report on patient outcomes, population health and social determinants of health to move toward Value-Based Payment; monitor and measure performance on HEDIS, UDS, PCMH and other clinical quality initiatives
  • Use data analytics to monitor and report statistical information for timely management decisions, contract negotiations, and as required for payers, federal and state agencies
  • Develop comprehensive work plans and strategies to accomplish goals and objectives
  • Assure that goals and objectives are monitored and adjusted when required; assure that data validation occurs by creating an auditing schedule for all quality measures; note and correct inaccuracies in a timely manner
  • Assist clinical departments by identifying trends in performance metrics; assist department leaders and staff in the development of action plans to improve data collection and outcomes, and resolve issues; conduct monthly rounds at all clinic sites, deliver reports, and conduct training to solidify all ongoing quality initiatives
  • Work closely with the Chief Medical Officer (CMO) to provide clinical data analysis to ensure the highest quality services are delivered to patients
  • Compile data, trend and report the highest clinical performers at Clinician Compliance Committee; identify Clinician champions for quality measures and work closely with champions to ensure quality measures meet performance thresholds per annual quality plan and CHAP
  • Facilitate and lead Clinical Outcomes Committee and serve as process improvement expert to all clinical subcommittees
  • Collaborate with CMO and Chief Compliance Officer to facilitate Quality Risk Management committee
  • Provide leadership and oversight for Quality Management activities throughout the organization
  • Provide direct leadership, support, education and training in corporate initiatives, such as patient safety and risk management as they pertain to quality
  • Participate in professional development activities to keep current with trends and practices in healthcare administration, quality management and data analytics; attends meetings, seminars, workshops and conferences, as needed and approved on behalf of the Clinic
  • Actively participate on various internal committees and external community initiatives as required or directed
  • Undertake special projects as requested

Qualifications Minimum

  • Bachelor’s Degree in nursing
  • Current California Registered Nurse license without restrictions
  • Minimum five years’ clinical experience
  • Minimum three years’ experience in successfully implementing performance improvement and quality programs in complex health care environment caring for the underserved and uninsured population
  • Experience/familiarity with computers, Microsoft Office products (Word/Excel/PowerPoint), business e-mail, communication systems and internet search capabilities and ability to quickly learn other software programs (Visio)

 Required Skills/Knowledge/Abilities

  • Ability to analyze reports and interpret data to develop and implement interventions to meet annual quality and organizational strategic goals.
  • Ability to proficiently develop in collaboration with IT a reliable analytics program to alert clinical staff to patients’ needed services
  • Possess subject matter expertise in HEDIS and UDS with intermediate knowledge of PCMH and grants management
  • Knowledge of community health centers, public assistance programs, state and federal regulations
  • Strong supervisory and administrative skills
  • Ability to provide leadership to the medical staff
  • Ability to engender trust and respect from the medical staff and professional community
  • Ability to accomplish organizational goals by managing medical and support staff
  • Excellent verbal and written skills necessary for communication with patients/clients, providers and other staff
  • Ability to interface with all levels of personnel in a professional manner and ability to work with people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs

Preferred Skills/Knowledge/Abilities

  • Master’s Degree in Public Health, Health Services Administration or equivalent
  • FQHC, Health Plan or IPA experience
  • Green Belt/Black Belt in LEAN Six Sigma
