ESSENTIAL DUTIES AND RESPONSIBILITIES:Assist patients to achieve an optimal level of function by facilitating timely and appropriate health services. To enable the patient to direct their care, self-advocate, and make decisions whenever possible. To identify cost-reduction opportunities and maintain cost-effectiveness in the provision of health services. To enhance productivity and satisfaction among members of the multidisciplinary team. To enhance patient satisfaction and retention. Apply nursing process (assessment, planning, intervention and evaluation) in managing caseload. Identifies the needs of the patient, to include medical, social, emotional and financial concerns through medical record review, consultation with patient, family and health care team members. Gather relevant data and obtain information by interviewing and performing careful investigation. Implement innovative nursing interventions, appropriate to the long-term needs of the patient, which complement and support the medical treatment plan, while integrating psychosocial, physiological and discharge planning needs. Manage care and cost of complex renal patients by exploring opportunities to reduce cost, decrease length of stay, reduce number of hospital re-admissions, enhance quality and coordinate care across entire care continuum. Provides patients, families, physicians, accounting department and hospital staff with information relating to insurance information coverage Coordinate health care services to ESRD patients across the continuum of care Identify and redirect practice patterns to facilitate care which effect appropriate care and cost-effective outcomes. Act as a resource in the development, and evaluation of renal treatment/practice guidelines. Evaluate and coordinate utilization of community resources available for renal patients and their families. Collaborate with Director of Health Service Coordination to develop, implement, and evaluate multidisciplinary patient/family educational programs related to chronic renal failure. Modify approach to care as appropriate to control quality/cost. Identify all existing and required clinical services in each market area. Facilitate patient/family access to necessary and appropriate health and human services. Uphold patient Bill of Rights and right to confidentiality.
