Position:Healthcare Coordinator

Healthcare Coordinators are responsible for clinical management of our Assisted Living community. The candidate will form lasting relationships with our residents, work a flexible schedule, and experience an environment with a warm, family atmosphere.

Healthcare Coordinator duties:

•  Provide proper documentation of all nursing and healthcare services

•  Responsible for maintaining health service plans and resident assessments

•  Monitor effective communications regarding resident care

•  Direct the department's safety and loss-prevention program

•  Supervise, evaluate and train Care Managers / Medication Care Managers and Wellness Nurses

•  Recruit and retain all team members in the department, ensuring their effective orientation and training

•  Establish positive relationship programs and practices

Requirements

Healthcare Coordinator requirements:

•  Must possess a degree from an accredited school of nursing

•  Current licensure as an RN

•  Minimum of 2 years' gerontological nursing and nursing administration experience

•  Ability to provide nursing services to the elderly while supervising a diverse staff
