HOME CARE HEALTH COORDINATOR

ACHIEVA is seeking an LPN to assess the service needs of families and schedule employees to provide in-home services for children and adults with mental retardation and other developmental disabilities. Responsibilities include effectively managing individual caseloads, acting as a liaison between the family, home care provider, contracting entity and the assigned case manager or supports coordinator. The ideal candidate will have satisfactorily completed a course in practical nursing, have graduated from an accredited and/or approved

LPN program, and have a current license in good standing as an LPN in the Commonwealth of PA. Prefer one-year previous experience with individuals with disabilities. Case management and/or home health experience helpful. Computer literacy required. Must have excellent organizational/interpersonal skills. Typical schedule Monday through Friday, business hours. Some local travel and occasional evenings and weekends required. Valid PA driver's license and insured vehicle required.
